Previous Grants

Previous CHC grants include:

  • Home Care Paraprofessional Training Programs in Spanish and Russian – a grant from The Cardinal Health Foundation enabled CHC to translate nine audio paraprofessional training Forum programs and corresponding workbooks into Spanish and Russian.

  • Depression in Older Home Health Patients – support through a grant from the New Your State Health Foundation enabled researchers from the Joan and Sanford I. Weill Medical College of Cornell University and four HCP member home care agencies to implement CAREPATH—a collaborative model designed to guide non-mental health nurses in evidence-based care of patients with depression in the home care setting.

  • Emergency Preparedness Grants from the New York State Department of Health (DOH) for 2010/11, 2009/10, 2008/09, 2007/08 and 2006/07 that help prepare home care agencies to handle emergencies including bioterrorism, natural disasters and pandemics.

  • Certified Diabetes Educator (CDE)-Home Care Paraprofessional Training Program from the New York State Health Workforce Retraining Initiative (HWRI) enables CHC to secure CDEs to provide on-site diabetes training for home health and personal care aides in three regions of New York State.

  • Retention Specialist Grant from the New York Health Foundation – a demonstration project that implements an evidence-based Retention Specialist model in 16 home care agencies to help improve retention of home care workers.
